I am registered with the ICO (Information Commissioners Office) which means I need to tell you what data I am collecting from you and what I intend to do with it.
What data do I keep and why do I need it?
Name – In order for us to communicate with one another.
email address and phone number – in order for us to have contact about your sessions.
Doctors Details – I will only use this information should there be a health emergency.
Session notes – I keep very brief anonymous notes of our session.
Will I share your data and if I do who will I share it with and for what purpose?
It is very unlikely that I will share your data. I will not sell it on or use it for unethical reasons. I may have to share it, if my notes are subpoenaed by court. If you or anyone you tell me about is at harm or risk of harm, I may have to pass this information on. I also have regular supervision where I talk about the work we are undertaking together but your identity is not disclosed to my Supervisor.
How will I store your data?
It is mainly all stored on paper in a locked cabinet. I use a password protected laptop for emails and your phone numbers may be kept in my mobile phone with your first name and last initial only.
How long will I store your data for and how will I dispose of it?
I will keep your session notes for 5 years. If under 18 years I will store the data for 5 years after your 18th birthday.
I will shred the document with your personal information on 1 month after our work finishes.
I will delete your phone number out of my mobile phone after our work finishes.
